Inquiring about Long-Term Care
When you are considering long-term care placement, the process can be overwhelming but we are here to help!
In order to be considered for long-term placement, we will need to obtain an admission packet which includes the following:
- Completed application and finance insert
- PRI/PASSR Screen
- History and Physical
- Medication List
- Nursing/MD Notes
- Social History
